Protecting Your Privacy

privacy Boomerang Workplace Wellness (BWW) is a national provider of wellness consulting and programming to corporations and individuals. We collect personal information from our clients in the regular course of doing business. Protecting our client’s privacy while handling their personal information is fundamental to the way we do business. This policy statement informs of our practices concerning the collection, use and disclosure of personal information and complies with applicable federal and provincial legislation including the Personal Information Protection and Electronic Documents Act (PIPEDA), an initiative designed to further protect the privacy of Canadian consumers.

Accountability

The personal information we collect, use and disclose is for the purpose of conducting the services which we provide.

BWW has an assigned a Privacy Officer, Colette Baser, to provide client service on matters of privacy relating to the collection, use and disclosure of personal information. Ms Baser is also responsible for maintaining this policy and updating staff on changes to both privacy legislation and business practices.

Purpose

We will collect, use and disclose information from questionnaires, application forms and other documents completed by you. In other instances we may collect your personal information directly from you during our interactions – whether via internet, by phone, or in person.

We collect personal information for different purposes depending on the type of service we are providing. These purposes include, but are not limited to:

  • providing quality preventative health screening services, lifestyle management services, and chronic disease reduction programs
  • enabling us to contact and maintain communication with our clients including, without limitation, to distribute preventative health related information
  • allowing us to effectively follow-up with our clients for services and/or participation in our programs

In order to provide certain services we may contract outside organizations or health professionals to perform specialized services such as collection of health screening biometric data. Our trusted service suppliers will be responsible for processing and handling the personal information they receive from you in a manner that is consistent with this policy.

When you visit our website we may use cookie and tracking technology depending on the features offered. Cooking and tracking technology are useful for gathering information such as browser type, operating system, tracking the number of visitors, or to further understand how visitors use our website. Cookies can also help us in customizing the website for our visitors. Personal information gathered from the website cannot be collected via cookies and other tracking technology. However, if you previously provided personally identifiable information, cookies may be tied to such information. Aggregate cookie and tracking information may be shared with third parties.

Consent

We require that consent to use personal information be obtained at the time of its collection. If BWW collects the personal information directly from an individual, or through an outside service supplier secured by BWW, we will obtain express consent for the collection, use and disclosure of such information.

Limiting Collection, Use, Disclosure and Retention

The amount and type of information gathered is limited to what is necessary for the identified purposes and is collected on a voluntary basis. Similarly, your personal information will not be used or disclosed for purposes other than those for which the information was collected.

We will retain your personal information only as long as it is required for the reasons it was collected. The length of time we retain information varies, depending on the product or service and the nature of the information.

When your personal information is no longer required we have procedures to destroy, delete, erase or convert it into an anonymous form.

Personal information for the purpose of this policy does not include information disclosed in the form of aggregate data. In this context, aggregate data refers to data that does not reveal and cannot be manipulated to reveal, person-specific information.

Accuracy

We will use our best efforts to ensure that your personal information is accurate, complete and up-to-date for the purposes that it is to be used. If any of your personal information changes, please inform us immediately so that we can make necessary changes.

Measures to Safeguard your Information

We use the following safeguards to maintain the confidentiality of your personal information:

1. Physical safeguards

  • Client information is not left unattended in areas to which the public has access
  • Privacy screens are used where necessary to prevent unauthorized individuals from viewing confidential information
  • When personal information is transported to another location it is sealed in an envelope, marked as confidential, and directed to the attention of the authorized recipient
  • Rooms and cabinets used to store personal information are locked when not in use and the distribution of keys is strictly controlled
  • Fax machines are located in a secure area. All fax transmissions are sent with a cover sheet that indicates the information being sent is confidential
  • Personal information in paper format is disposed of by confidential shredding

2. Administrative safeguards

  • Only the least amount of information necessary for the intended purpose is collected, used and disclosed
  • Access to personal information is restricted to staff who require access to the information in order to perform their duties
  • All staff, contracted persons and temporary staff are required to sign a confidentiality agreement
  • Conversations regarding confidential information is protected in a public environment by ensuring the conversation cannot be overheard or intercepted

3. Technological safeguards

  • Screen saver passwords are used to protect against unauthorized access if computer is left unattended for 2 minutes or more
  • Passwords are kept confidential at all times and are not written down or shared with other staff, and are changed every 3 months
  • Firewalls to protect unauthorized access are in place
  • System backups are done on a weekly basis

Individual Access

At your written request we will inform you whether we hold personal information relating to you and if we do, we will let you know the purposes for which we used your personal information. If your request is made in writing, and subject to applicable laws, we will provide you with access to your personal information that is in our possession. You will be required to provide information or documentation to verify your identity.

Changes to this Policy

It is our practice to review our policies periodically therefore we may amend our Privacy Policy in the future. It is your responsibility to review this policy and upon initial consent to participate in a BWW program, you will be deemed to have consented to any changes to this Privacy Policy, whether or not you are otherwise notified or aware of such changes.
